Zusammenfassung

Die Aortenklappeninsuffizienz (AI) ist definiert als Unfähigkeit der Aortenklappe zu einem vollständigen diastolischen Klappenschluss. Die AI ist entweder primär durch eine Pathologie der Aortenklappe bedingt oder sekundär durch Veränderungen der Aortenwurzel bzw. der Aorta thoracalis ascendens. Es wird unterschieden zwischen der akuten Verlaufsform, bei der es rasch zur Entwicklung von Symptomen der akuten Linksherzinsuffizienz kommen kann, und der chronischen Verlaufsform, bei der die AI oft lange kompensiert ist und es erst spät mit einer Dilatation und Einschränkung der Funktion des linken Ventrikels zur Linksherzinsuffizienz kommt. Bei asymptomatischen Patienten ohne Zeichen der Dilatation oder Funktionseinschränkung des linken Ventrikels erfolgt eine konservative Behandlung, wohingegen bei symptomatischen Patienten mit hochgradiger AI oder Zeichen der linksventrikulären Schädigung die Indikation zur operativen oder interventionellen Behandlung der Aortenklappe besteht.
