Zusammenfassung
Die Aorteninsuffizienz kann in eine seltene angeborene und eine häufigere erworbene Form unterschieden werden. Eine Endokarditis, die meist rheumatischer oder bakterieller Genese ist, kann die Aortenklappe so schädigen, dass sich akut eine Aorteninsuffizienz entwickelt. Es können aber auch degenerative Klappenveränderungen oder eine Aortendissektion ursächlich für eine Aorteninsuffizienz verantwortlich sein. Anfangs häufig unbemerkt kommt es in fortgeschrittenen Stadien zu einer Linksherzinsuffizienz mit Dyspnoe als Leitsymptom. Zur Diagnostik gehören Blutdruckuntersuchung (große Blutdruckamplitude mit niedrigem diastolischen Blutdruckwert), EKG (linksventrikuläre Hypertrophie mit einem positiven Sokolow-Lyon-Index), Röntgen-Thorax (dilatierter, aortal konfigurierter linker Ventrikel) und Farbdopplerechokardiografie (diastolischer Rückfluss in den linken Ventrikel), ggf. auch eine transösophageale Echokardiographie und eine Herzkatheteruntersuchung. Die Behandlung besteht aus Nachlast senkenden Medikamenten bei arteriellem Hypertonus, einem Diuretikum bei Flüssigkeitsretention und Herzinsuffizienzsymptomatik sowie im Einzelfall eine operative Versorgung mittels Aortenklappenersatz oder Klappenrekonstruktion.
