Zusammenfassung

Die akute Appendizitis bleibt ein spannendes Krankheitsbild. Klinische Symptome und Verlauf weisen große Variabilität auf und unterliegen altersabhängigen Besonderheiten. Die Diagnostik wird heute neben der klinischen Untersuchung von einer qualifizierten Sonografie geprägt, die zusammen mit einer Labor-Basisdiagnostik über den Interventionsbedarf zu entscheiden, aber auch mögliche Differenzialdiagnosen zu klären hat. Therapeutisch konkurrieren nicht nur operative Verfahren und Zugänge (offene vs. laparoskopische Operation), sondern auch Empfehlungen einer primär antibiotischen Therapie mit oder ohne Appendektomie im symptomfreien Intervall. Die Krankheitsschwere variiert auch heute, wenngleich für die Mehrzahl betroffener Patienten und unabhängig vom gewählten operativen Vorgehen ein zunehmend kürzerer stationärer Aufenthalt resultiert.
