Zusammenfassung

Medikamentenabhängigkeit im Rahmen der medikamentösen Therapie von Schmerzerkrankungen ist ein hochaktuelles und wichtiges Thema. Dies betrifft vor allem Opioide, aber auch Pregabalin und Cannabinoide unter der Zielsetzung analgetischer Wirksamkeit, sowie die Behandlung von Schmerzen bei komorbiden Substanzabhängigkeiten. Aktuelle Begrifflichkeiten und Definitionen der Medikamentenabhängigkeit und des -fehlgebrauchs werden dargestellt. Eine Epidemie von Abhängigkeitserkrankungen als Folge schmerzmedizinischer Behandlung findet sich in Deutschland nicht, aber durchaus in anderen Ländern mit hohen Opioidverordnungen. Grundsätze einer rationalen Steuerung medikamentöser Schmerztherapie liegen in Form praktisch orientierter Leitlinien vor. Ein begleiteter Medikamentenentzug ist im schmerzmedizinischen Behandlungskontext ambulant, teilstationär und stationär etabliert und praktikabel. Für Patienten mit führender Suchtdynamik ist eine enge Abstimmung mit suchtmedizinischer Expertise sinnvoll.
