Zusammenfassung
Die X-chromosomal vererbte angeborene Hämophilie A (Faktor-VIII-Mangel) und Hämophilie B (Faktor-IX-Mangel) führen abhängig vom Schweregrad zu Blutungen perioperativ oder bei Bagatellverletzungen, bei schwerer Hämophilie treten spontane Blutungen vor allem in Gelenke auf. Die Therapie erfolgt durch Substitution des fehlenden Gerinnungsfaktors, zur Prophylaxe von Blutungen steht auch ein bispezifischer Antikörper bei Hämophilie A oder eine Gentherapie zur Verfügung. Betroffene mit schwerer Hämophilie oder schwerem Blutungsphänotyp werden prophylaktisch behandelt. Komplikationen sind die hämophile Arthropathie und die Entwicklung von Inhibitoren gegen den substituierten Gerinnungsfaktor. Die erworbene Hämophilie entsteht bei zuvor gerinnungsgesunden, meist älteren Menschen durch Autoantikörper gegen Faktor VIII und führt in der Regel zu schweren Blutungen. Die Therapie von Blutungen ist mit Bypasspräparaten möglich, zur Eradikation des Inhibitors wird eine Immunsuppression eingesetzt.
