Abstract

In dit hoofdstuk leert u meer omtrent de werkwijze van de hartfalenpolikliniek en de hartfalenverpleegkundige. De hartfalenverpleegkundige functioneert als laagdrempelig aanspreekpunt voor de patiënt en zal vaak intermediair zijn tussen patiënt en cardioloog.
