Riassunto

L’importanza di una corretta diagnosi prenatale delle lesioni espansive del collo è primariamente correlata con la possibile compressione delle vie aeree da parte della massa. Per tale ragione è fondamentale valutare l’anatomia delle vie aeree e identificare l’eventuale parziale o completa ostruzione delle stesse, al fine di programmare, se necessario, un trattamento EXIT (Ex Utero Intrapartum Treatment). L’ecografia prenatale è in grado di identificare la lesione, di definirne la struttura e la vascolarizzazione. Attraverso segni indiretti, come la ridotta deglutizione fetale, il polidramnios e la protrusione della lingua, è possibile riconoscere l’ostruzione delle vie aeree [1, 2]. L’ecografia ha tuttavia dei limiti nel valutare l’estensione profonda della massa e nel visualizzare direttamente la laringe e la trachea. Nelle fasi avanzate della gravidanza essa è limitata dalla posizione del feto e degli arti, dall’avanzata fase di ossificazione delle ossa del cranio e del massiccio facciale; inoltre, eventuali anomalie fetali possono esse stesse rappresentare un ostacolo all’esame.
