Zusammenfassung
Schwester und Bruder, zweieiige Zwillinge, beide Typ-1-Diabetes seit Jugend, stellen sich im 31. Lebensjahr vor, da beide einen HbA1c von 7,8 % haben, beim Versuch ihn besser einzustellen immer wieder Unterzuckerungen auftreten und bei beiden, trotz normaler Blutdruckwerte, eine Albuminurie aufgetreten ist. Nun wollen sie aufgeklärt werden, ob sie das Risiko der niedrigeren HbA1c-Einstellung auf sich nehmen sollten, eventuell eine Pumpentherapien versuchen sollen und ob mit Sicherheit die Senkung des HbA1c in den Normbereich die Nierenschädigung reversibel macht oder zumindest aufhält. Was lohnt sich für wen?
