Zusammenfassung
Die Hämophilie A wird X-chromosomal rezessiv vererbt. Deswegen erkranken fast ausschließlich Männer. Frauen bleiben aufgrund ihres zweiten „gesunden“ X-Chrompsöms bis auf extrem seltene Ausnahmen symptomlos und geben als Überträgerinnen (Kondüktorinnen) die Erbanlage für die Hämophilie an die Hälfte ihrer Söhne und Töchter weiter.
Frauen gelten als gesicherte Überträgerinnen, wenn der Vater an einer Hämophilie A leidet, oder wenn sowohl in früheren als auch in nachfolgenden Generationen der mütterlichen Linie jeweils mindestens ein Hämophiler bekannt ist. Bis vor kurzem galten Frau auch als sichere Überträgerinnen, wenn sie mehrere hämophile Söhne hatten. Diese Aussage muß man nach heutigen Erkenntnissen relativieren, weil in diesen Fällen auch eine Mosaikmutation vorliegen kann. Eine Mosaikmutation entsteht nach der Zygoten-bildung und betrifft nur einen Teil der Körperzellen. Das Wiederholungsrisiko für einen weiteren hämophilen Sohn ist dann abhängig vom Anteil der Ovarialzellen, die den Gendefekt trafen.
