Zusammenfassung
Experimentelle und klinische Untersuchungen haben gezeigt, daß Salpetersäureester (organische Nitrate) bei akutem Myokardinfarkt die linksventrikuläre Kontraktions-funktion verbessern und zu einer geringeren Größe des infarzierten Myokardbereiches führen [1-3]. Neuere Untersuchungen haben darüber hinaus ergeben, daß Nitrate neben der vasodilatierenden Wirkung auch die Thrombozytenfunktion hem men [4]. Dies und der bisher nur im Tierexperiment erhobene Befund, daß Nitrate die Akkumulation neutrophiler Granulozyten nach Koronarokklusion und Reperfusion herabsetzen [5], konnte zur Verbesserung der koronaren Mikrozirkulation am reperfundierten ischämischen Herzen und damit zur antiischämischen Wirksamkeit dieser Substanzklasse beitragen.
