Zusammenfassung
Epiphysenfrakturen des Wachstumsalters unterliegen im klinischen Alltag einer der Röntgenmorphologie angepaßten Einteilung nach Aitkin et al. [1] oder Salter u. Harris [14]. Entsprechend dem Verletzungsmuster des Knorpels werden prognostische Parameter aus der Art des Bruches abgeleitet (Abb. 1). Ein vorzeitiger Verschluß der Wachstumsfuge wurde insbesondere von Salter et al. [14] auf eine direkte Verletzung des Stratum germinativum des Epiphysenknorpels zurückgeführt und daraus als therapeutische Konsequenz eine invasive Therapie mit „wasserdichter“ Rekonstruktion der Epiphyse gefordert. Im Bereich der distalen Tibia erhöhte sich dieser Anspruch an den Therapeuten, da die als prognostisch ungünstig eingestuften Frakturtypen (Aitken III, Salter III/IV) eine Beteiligung der Gelenkfläche an einer belasteten Extremität beinhalteten. Insbesondere im deutschsprachigen Schrifttum wurde aus diesem Zusammenhang die Indikation zur offenen Reposition sehr weit gestellt [11, 17], während in der englischen Literatur bei Bevorzugung der konservativen Therapie die Diskussion sehr kontrovers geführt wird [4–6, 15].
