Zusammenfassung
In diesem Kapitel wird die Rolle der objektiven audiometrischen Verfahren bei der präoperativen Diagnostik (Eignungstests und Ausschlussbedingungen) vor Versorgung mit einem Cochlea-Implantat und bei der postoperativen Erfolgskontrolle dargestellt. Die audiologische Indikation zur Implantation verlangt den Nachweis erstens eines profunden Funktionsdefizits des Innenohres und zweitens der erhaltenen Hörnervenfunktion. Mit Hilfe von OAE, ECochG, BERA und ASSR gelingt die Bestimmung der (unversorgten) Schwelle mit ausreichender Zuverlässigkeit. Bis auf den Sonderfall der AS/AN fehlen die Antworten ganz (OAE) oder sie sind nur bei sehr hohen Reizpegeln auslösbar. Für den präoperativen Nachweis von elektrisch evozierten Potenzialen des Hörsystems steht kein routinetaugliches Verfahren zur Verfügung. Postoperativ gelingt dies regelmäßig durch Reizung mit den Elektroden des Implantats und Ableitung ebenfalls mit diesen (ECAP) oder mit EEG-Oberflächenelektroden (E-BERA). Diese Antworten sind Grundlage der Anpassung des Sprachprozessors und können auch zur Evaluation des Erfolges einer CI-versorgung herangezogen werden. Mit den für das Kindesalter typischen Problemen und Einschränkungen können hierfür auch kortikale Potenziale (E-CERA) genutzt werden.
