Zusammenfassung

Urothelkarzinome des oberen Harntraktes finden sich hauptsächlich bei älteren Patienten (medianes Alter 70 Jahre). Männer sind 3- bis 4-mal häufiger betroffen als Frauen. Risikofaktoren sind heute vor allem das Zigarettenrauchen, die Aristolochiasäure-Nephropathie, das Urothelkarzinom der Harnblase und das Lynch-Syndrom. Historisch spielen auch aromatische Amine und Phenacetin eine Rolle. Hämaturie gefolgt von Flankenschmerzen häufigsten Symptome bei der Diagnostik eines Urothelkarzinoms des oberen Harntraktes. Andere klinische Symptome wie palpable Raumforderungen oder stark hydronephrotisch veränderte Nieren sind äußerst selten. Asymptomatische Tumoren werden in 10–15 % der Fälle beobachtet. Die klinische Risikostratifikation ist schwierig. Die besten Ergebnisse erhält man bei Kombination aus: Biopsie, Grading, Tumorarchitektur, Zytologie, Hydronephrose, Multifokalität und Tumorgröße.
