Zusammenfassung
Toxische oder unerwünschte Arzneimittelwirkungen (UAW) sind oft bei einer akuten neuropsychiatrischen Symptomatik mitbeteiligt, gelegentlich auch ausschlaggebend. Sie reichen von Befindlichkeitsstörungen bis zu ausgeprägten zerebralen Allgemeinsyndromen einschließlich Koma. Wegen ihrer Reversibilität und der begleitenden motorischen und vegetativen Symptome ähneln sie den metabolischen Enzephalopathien. Auch prolongierte toxische, meist atrophisierende ZNS-Gewebereaktionen kommen als Medikationsfolge vor (z.B. Methotrexat). Viele zentrale Wirkungen sind im EEG erkennbar (Akzentuierungen im Beta-Spektrum durch Benzodiazepine und Barbiturate, im Theta-Delta-Band durch Neuroleptika, triphasische Wellen durch z.B. Cephalosporine). Werden bestimmte Rezeptoren blockiert oder exzessiv stimuliert, erscheint dies klinisch als sog. Toxidrom. Unterschieden werden das Hypnotika- und Opiatsyndrom, das sympathikolytische und -mimetische Syndrom sowie das cholinerge, zentral anticholinerge Syndrom, das Serotonin-Syndrom und das Dopamin-Entzugssyndrom („malignes neuroleptisches Syndrom“) und hier bezüglich Diagnose und Therapie detailliert besprochen.
