Zusammenfassung
Neben den naheliegenden enzephalopathischen Mangelerscheinungen im Rahmen von Malnutrition, Maldigestion oder Malabsorption bei Darmerkrankungen spielen auch autoimmunologische Aspekte (Kreuzantigene, vaskulitische Mechanismen) und neuroinvasive infektöse Enzephalopathien eine Rolle. Beispiele sind der M. Whipple, die Zöliakie und die inflammatorischen Darmerkrankungen wie Colitis ulcerosa und M. Crohn, wobei letztere auch zu vermehrten Schlaganfällen disponieren. Das zerebrale Allgemeinsyndrom kann sich hier neben Appetit- und Verdauungsstörungen sowie Nahrungsintoleranz auch durch die typischen hämatologischen und dermatologischen Mangelsymptome bemerkbar machen, gelegentlich auch durch Mangel-Polyneuropathien. Pathognomonisch sind Hirnstammsymptome bei der Thiaminmangel bedingten Wernicke-Enzephalopathie (WE, Ataxie, Okulomotorikstörungen und amnestisches Psychosyndrom) und beim M. Whipple (okulo-mastikatorische Myorhythmien mit Pendelnystagmus). Besteht der geringste Verdacht auf eine Wernicke-Enzephalopathie, ist die parenterale Thiaminsubstitution angezeigt, um schwere hippokampal betonte Nekrosen und ein Korsakow-Syndrom zu vermeiden.
