Effetti metabolici della chirurgia bariatrica

La chirurgia bariatrica, nata come terapia “estrema” dell’obesità, ha mostrato, in un continuo crescendo di evidenze scientifiche, uno spettro di effetti benefici duraturi su tutte le componenti della sindrome metabolica, attraverso meccanismi d’azione, almeno in parte indipendenti dagli esiti ponderali, e ad oggi non ancora interamente noti, guadagnando così la dizione di “chirurgia metabolica” (Fig. 1) (Rubino et al. in Ann. Surg. 259(1):117–122, 2014). Su tale base appare, pertanto, evidente la necessità di inserire appropriatamente tale strumento terapeutico nell’ambito di percorsi non solo mirati agli esiti “bariatrici”, ma nell’ambito più esteso dei dismetabolismi della sindrome metabolica.
