In-vitro-Fertilisation und intrazytoplasmatische Spermieninjektion

Zusammenfassung

Konnten anfänglich nur Paare mittels konventioneller IVF behandelt werden, bei denen eine tubare oder unerklärte Sterilität vorlag, erlaubte die Entwicklung invasiverer Befruchtungstechniken (z. B. der ICSI) letztendlich auch eine Therapie bei ausgeprägtem männlichem Faktor. Erst nach der Gewinnung der Cumulus-Eizell-Komplexe wird anhand der Samenqualität des Partners entschieden, welche Technik angewandt wird. Die herkömmliche IVF ist im Wesentlichen der in vivo stattfindenden Befruchtung nachempfunden, d. h., in einem Kulturschälchen werden die Eizellkomplexe mit einer festgelegten Zahl an Spermien versetzt und traditionellerweise über Nacht inseminiert. Bei einer ICSI hingegen müssen vor der geplanten Injektion die die Eizelle umgebenden Cumuluszellen enzymatisch/mechanisch entfernt werden. Das mittels unterschiedlichster Präparationstechniken gewonnene Spermium muss vor Verwendung immobilisiert werden.
