Nichtgerminale Hodentumoren

Zusammenfassung

90 % aller Hodentumoren gehen von den Keimzelltumoren aus. Die restlichen 10 % aller testikulären Neoplasien gehen histologisch aus sieben verschiedenen Gewebsabschnitten des Hodens hervor. Am häufigsten sind mit 38 % aller nichtgerminalen Tumoren (4 % aller Hodentumoren) die malignen Lymphome. Dies sind überwiegend B-Zell Lymphome, von denen ein Drittel beidseitig auftritt. Nach Ablatio testis ist eine Anthrazyklin-haltige Chemotherapie erforderlich. Die mittlere 5-Jahresüberlebensrate liegt bei etwa 60 %. Keimstrang/Stromatumoren sind mit 26 % die zweithäufigste Gruppe der nichtgerminalen Tumoren. Hierzu zählen die Leydigzelltumoren, die seltenen Sertolizelltumoren und die extrem seltenen Granulosazelltumoren. Mehr als 90 % dieser Tumoren sind gutartig und können durch organerhaltende Exzision behandelt werden. Aufgrund der Seltenheit dieser Tumoren gibt es keine Leitlinien-basierten Therapieempfehlungen. Bei den seltenen malignen Verlaufsformen dieser Tumoren ist die retroperitoneale Lymphadenektomie indiziert. Alle übrigen Entitäten aus der Gruppe der nichtgerminalen Tumoren gelten als extreme Raritäten.
