Zusammenfassung
Die medikamentöse Behandlung des Urothelkarzinoms ist ein wesentlicher Bestandteil der Therapie. Insbesondere bei den lokal fortgeschrittenen Tumorstadien und im Falle einer Metastasierung, aber auch in der neoadjuvanten und adjuvanten Situation, spielt die Systemtherapie eine zentrale Rolle.
Die Entwicklung neuer Substanzen hat das Armamentarium der Systemtherapie des Urothelkarzinoms deutlich erweitert. Durch die immunmodulatorische Therapie steht zudem ein gänzlich anderer Wirkmechanismus zur Verfügung. Im Einzelfall sind somit auch bei einer fortgeschrittenen Erkrankung optimistisch stimmende Krankheitsverläufe zu beobachten.
In dem vorliegenden Kapitel werden die aktuell zugelassenen und bedeutenden systemischen Behandlungsoptionen tabellarisch beschrieben.
Die Diagnostik und Indikationsstellung sowie die Behandlung maligner Tumore mit Zytostatika birgt letale Risiken und muss durch einen hämatologisch/onkologisch erfahrenen Arzt erfolgen. Indikation, Kontraindikationen, Dosierung und Applikation muss unter Berücksichtigung der Fachinformation des Herstellers eigenverantwortlich überprüft und an die klinische Situation angepasst werden. Die angegebenen Protokolle lassen nicht auf eine generelle Zulassung schließen.
