Zusammenfassung
Angeborene ösophageale Fehlbildungen gelten als kinderchirurgische Indexdiagnosen. Von besonderer Bedeutung ist die Ösophagusatresie mit ihren Unterformen. Die rekonstruktive Ösophaguschirurgie steht im Vordergrund der kinderchirurgischen Behandlung und dem Erhalt des eigenen Ösophagus kommt eine wichtige Bedeutung zu. Allerdings ist das dauerhafte funktionelle Ergebnis der wichtigste Parameter für den chirurgischen Erfolg. Nach der primären Ösophagusanastomose können sowohl Frühkomplikationen (Leckage, Anastomosenstenose, Rezidiv der tracheoösophagealen Fistel) als auch Spätkomplikationen (gastroösophagealer Reflux, Motilitätsstörung, Tracheomalazie) auftreten. Diagnose und Behandlung dieser Komplikationen sind für den Langzeitverlauf genauso wichtig wie der Operationserfolg. Noch seltenere Krankheitsbilder stellen die Laryngotracheoösophageale Spalte, die angeborene Ösophagusstenose und die ösophageale Duplikatur dar. Hier ist eine interdisziplinäre Diagnostik und Therapie essenziell.
