FormalPara Synthese – Verteilung – Abbau – EliminationTheophyllin wird hepatisch zu 3-Methylxanthin bzw. 1,3-Dimethylharnsäure und weiter zu 1-Methylharnsäure metabolisiert, sodass sich im Urin im Wesentlichen Abbauprodukte finden. Bei Neugeborenen (Indikation: Apnoebehandlung) wird Theophyllin zusätzlich zu Coffein methyliert.
FormalPara HalbwertszeitIm Plasma Erwachsener 6–9 Stunden, von Kindern und Rauchern 4 Stunden, von Frühgeborenen 30 Stunden.

Therapeutischer Bereich (S, P): 8–20 mg/L (Erwachsene), 6–11 mg/L (Frühgeborene); toxisch: >20 mg/L (Erwachsene), >15 mg/L (Frühgeborene); komatös-letal: >50 mg/L (Erwachsene), unbekannt (Frühgeborene).
