FormalPara Synthese – Verteilung – Abbau – EliminationDie Zufuhr erfolgt in der Regel intravenös in sehr unterschiedlicher Dosierung jeweils gemäß einem individuellen Behandlungsprotokoll. Methrotrexat gelangt mittels eines plasmamembranständigen Transportsystems in die Zelle und wird dort gespeichert. Es wird überwiegend unverändert renal eliminiert.
FormalPara HalbwertszeitInitial: 2–4 Stunden (Plasma); terminal: 8–15 Stunden (Plasma).
FormalPara Funktion – PathophysiologieMethotrexat ist ein Antimetabolit, der Dihydrofolsäure kompetitiv von der Dihydrofolatreduktase verdrängt. Es entsteht ein Mangel an Tetrahydrofolsäure für die Thymidylatsynthese und damit eine Störung der Zellteilung.

FormalPara Diagnostische WertigkeitDer therapeutische Bereich ist abhängig vom Behandlungsschema. Bei Überschreitung des Referenzbereichs wird Calciumfolinat (sog. „rescue“) gegeben, das die Zytotoxizität von MTX gegenüber den normalen Zellen vermindert.
