Zusammenfassung

Zahlreiche Erkrankungen können eine intra-abdominellen Hypertension oder manifestes abdominellen Kompartmentsyndrom (ACS) verursachen. Dem Gefäßchirurgen begegnet das ACS meist im Rahmen des rupturierten Bauchaortenaneurysmas. Die Prävalenz liegt hier bei 6,8–8 %. Die Auswirkungen des erhöhten intra-abdominellen Drucks sind vielfältig und betreffen zahlreiche Organsysteme. Die Diagnose beruht auf der klinischen Einschätzung und der Messung des intra-abdominellen Drucks über einen Blasenkatheter. Liegt dieser dauerhaft über 20 mmHg und geht mit einer neu aufgetretenen Organdysfunktion einher, liegt definitionsgemäß ein abdominelles Kompartmentsyndrom vor. Da die Letalität des manifesten ACS mit 39–62 % auch bei zeitgerechter, aggressiver Therapie (operative Dekompression) sehr hoch ist, kommt der konservativen Behandlung mit Senkung der intra-abdominellen Hypertension eine überragende Bedeutung zu.
