Besonderheiten der Pharmakotherapie bei geriatrischen Patienten

Zusammenfassung

Ein geriatrischer Patient zeichnet sich gleichermaßen durch Multimorbidität und die Gebrechlichkeit aus. Im Alter bestehen Besonderheiten, die insbesondere die Pharmakokinetik und Pharmakodynamik (und damit die Elimination und Wirkstärke) und die zunehmende Multimorbidität betreffen. Die Applikation eines oder mehrerer Medikamente kann mit einer der Erkrankungen eines multimorbiden geriatrischen Patienten interagieren bzw. die Pharmakokinetik und Pharmakodynamik eines Arzneimittels beeinflussen. Im Alter ist zum einen die Nierenfunktion und die Leberfunktion eingeschränkt, zum anderen kommt es zu einer altersabhängigen Veränderungen der Verteilung eines Arzneimittels im Körper. Auch sind die Gegenregulationsmechanismen bei einem Blutdruckabfall vermindert. Die Folge ist eine deutlich erhöhte Rate unerwünschter Arzneimittelwirkungen und -interaktionen bei älteren Patienten. In der Beers-Liste 2012 und der PRISCUS-Liste sind die potenziell gefährlichen Pharmaka bei geriatrischen Patienten zusammengefasst und Alternativen aufgezeigt.
