Zusammenfassung
Tumoren des Analkanals gehen meist vom Plattenepithel aus sund sind häufig mit eine HPV-Infektion assoziiert. Die Entstehung von Plattenepithelkarzinomen verläuft über prämaligne Vorstufen, die analen intraepithelialen Neoplasien. Eine rektale Blutung stellt das häufigste Erstsymptom, hinzu kommen eine tastbare Raumforderung und ggf. Schmerzen. Grundlage der Diagnostik bildet die Anamnese und die körperliche Untersuchung (Inspektion des Lokalbefundes, digitale Untersuchung, Lymphknotenschwellungen). Eine Proktoskopie ist unabdingbar. Die Ausbreitungsdiagnostik umfasst neben Sicherung von Größe und Infiltrationstiefe des Tumors auch die Erfassung möglicher Fernmetastasen und Lymphknotenmetasten. Für die lokoregionäre Ausbreitungsdiagnostik ist die MRT ergänzt um die Endosonographie die Methode der Wahl. Die Behandlung erfolgt stadienabhðngig. In den lokalisierten stadien erfolgt in der Regel eine Radiochemotherapie mit Mitomycin C und 5-Fluorouracil. Eine intensitätsmodulierte Strahlentherapie kann bei Einbeziehung von befallenen Lymphknoten zu einer besseren Schonung von gesundem Gewebe beitragen. In fortgeschrittenen Stadien ist die Prognose ungünstig, die Therapie in der Regel palliativ.
