Zusammenfassung
Die Arthrose des Sternoclaviculargelenks (SCG) stellt eine häufige Pathologie dar und kann bei 60-jährigen Patienten in bis zu 60 % der Fälle nachgewiesen werden. Während die Erkrankung häufig asymptomatisch verläuft und somit keiner Therapie bedarf, gibt es andererseits Verläufe mit starker Schmerzhaftigkeit und konsekutiver Einschränkung der Schulterfunktion. In diesen seltenen Fällen kann nach Ausschöpfung der konservativen Möglichkeiten eine operative Therapie notwendig werden. Bei der Operation, die offen oder arthroskopisch durchgeführt werden kann, werden die zerschlissenen Discusanteile entfernt, die arthrotischen Anteile der medialen Clavicula und des Sternum reseziert (ca. 8 mm) und die Kapsel stabil wieder verschlossen. Die postoperativen Ergebnisse der SCG-Resektion sind in der Literatur als verlässlich und gut bis sehr gut beschrieben. Aufgrund der wichtigen anatomischen Strukturen, die direkt retrosternal gelegen sind, sollte dieser Eingriff nur von Chirurgen mit Erfahrung auf dem Gebiet der Chirurgie am Sternoclaviculargelenk und exakter Kenntnis der Anatomie in diesem Bereich durchgeführt werden.
