Zusammenfassung
Unter allen onkologischen Erkrankungen im Kindes- und Jugendalter stellen die primären ZNS-Tumoren mit ca. 25 % die zweithäufigste Gruppe dar. Die primäre Resektion ist meist von großer Bedeutung, da die Patienten häufig durch die lokale Raumforderung oder einen Verschlusshydrozephalus vital bedroht sind. Die Resektion sollte immer nur so weit wie verantwortbar und unter Vermeidung weiterer neurologischer Ausfälle erfolgen. Von jedem ZNS-Tumor sollte, wenn klinisch vertretbar, ausreichend Formalin-fixiertes, Paraffin-gebettetes und frisch gefrorenes Material asserviert werden. Als Vergleichsgewebe für molekulare Tumoranalysen dient EDTA-Blut, wobei eine genomweite Methylierungsarrayanalyse und/oder eine Neurogenpanelanalyse als Standard gelten. Immer sollte zusätzlich Liquor asserviert werden. In Abhängigkeit von der Entität, dem Vorliegen eines Prädispositionssyndroms und dem Resektionsausmaß kommen eine systemische, ggf. auch intraventrikuläre Chemotherapie und eine lokale und/oder kraniospinale Radiotherapie zum Einsatz. Zielgerichtete Therapien sind heute noch die Ausnahme, werden bei besserem Verständnis der molekularbiologischen Veränderungen in Zukunft an Bedeutung gewinnen. Die Aufsplitterung der Entitäten in immer kleinere, prognostisch unterschiedliche Subgruppen erfordert neben standardisierter Diagnostik, eine stratifizierte, risikoadaptierte Therapie, die bei kleinen Fallzahlen eine zunehmende nationale und internationale Vernetzung der translationalen und klinischen Forschungsgruppen und Versorgern bedingt.
