Zusammenfassung
In der modernen Patientenversorgung, nicht nur in Krankenhäusern, sondern auch in Altenpflegeheimen stellen nosokomiale Infektionen eine zunehmende Herausforderung dar. In einem Bericht des Europäischen Zentrums für Krankheitsprävention und -kontrolle (ECDC) wird geschätzt, dass einer von 20 Krankenhauspatienten eine nosokomiale Infektion erleidet (Conklin et al. 2008). Sowohl in monetärer (erhöhte direkte und indirekte Kosten) als auch in klinischer Hinsicht (erhöhte Letalität und Morbidität) verursachen nosokomiale Infektionen große Schwierigkeiten (De Angelis et al. 2010; Plowman 2000). In den letzten Jahren ist daher zunehmend der Gedanke der Infektionsprävention in den Vordergrund getreten, also die Verhinderung von Infektionen vor deren Auftreten und vor allem vor deren Weitergabe, also Verhinderung von Transmissionen. Änderungen in der Epidemiologie von Wirt und Erreger – verstärkt vulnerable Patienten, Zunahme an multiresistenten Erregern (MRE), Fehlen von neuen therapeutischen Substanzen – bedingen auch die erhöhte und gewandelte Transmissionsdynamik von nosokomialen Infektionen. Besonders im Zuge des demographischen Wandels sind Ärzte aktuell mit zunehmend älteren und häufig komorbiden Patienten konfrontiert, die vulnerabel für das Erleiden einer Infektion sind. Zukünftig werden also gerade ältere Patienten die Hauptgruppe in der Patientenversorgung in den meisten medizinischen Einrichtungen darstellen. Durch die Vulnerabilität in Bezug auf Infektionen bei geriatrischen Patienten (Immunoseneszenz, physiologische Veränderungen im Alter, funktionelle Einschränkungen und Komorbiditäten), der vermehrte Einsatz von Devices sowie die Zunahme von MRE wird die Bürde von Infektionen im Kollektiv der älteren Patienten zukünftig also eher steigen. Reha-Einrichtungen und Langzeitpflegeeinreichtungen rücken daher zu Recht in den krankenhaushygienischen Fokus.
