Abstract
Growing numbers of frail, elderly patients with multiple hospital admissions are a major challenge facing healthcare. Anticipatory care to avoid hospitalisation for these patients involves identification and proactive intervention. A rapid response when patients are acutely admitted to a hospital to promote rapid assessment and discharge reduces the time spent in hosptial. This chapter studies developments in this area and provides a guide and suggestions on how to care for an increasingly frail population in the community. This ought to reduce the burden on hosptial care and reduce unnecessary admissions, particularly when providing end of life care.
