A 48-year-old man developed severe hypoxemia 48 h after right pneumonectomy with chest wall resection for primary bronchogenic carcinoma. A large right upper lobe mass abutting the chest wall is seen on baseline chest radiograph and CT (a, b). Two days later, diffuse ground-glass opacities are noted throughout the left lung (c–e). Postoperative changes related to right pneumonectomy and partial right lateral chest wall resection, including a large amount of subcutaneous emphysema, are also seen. A large amount of subcutaneous emphysema is noted on the right. The patient remained ventilator dependent and succumbed to progressive respiratory failure
