Definition
The life care planning (LCP) process uses a systematic methodology to identify and quantify the multidimensional disability-related needs of an individual who has experienced catastrophic injury or who needs significant disability-related services throughout his or her life. The results of this process are summarized in a life care plan, which has been defined as “a dynamic document based on published standards of practice, comprehensive assessment, data analysis, and research, which provides an organized concise plan for current and future needs with associated costs, for individuals who have experienced catastrophic injury or have chronic health care needs (Weed and Berens 2010).” The purpose of a life care plan is to serve as a “road map” to inform the person with a disability, her or his family members, and service providers about anticipated disability-related needs and services necessary (at which points in time) to meet those needs.
