Introduction

Nursing homes provide short-term postacute skilled nursing facility and rehabilitative care after hospital discharge. They also provide care to those in need of long-term services and support as a result of frailty, disability, functional decline, and cognitive deficit. Ensuring quality care in nursing homes poses a particular problem for policymakers. People served by nursing homes are by definition vulnerable and poorly positioned to advocate for themselves; more than half are cognitively impaired. While family members can act as advocates for some, many families are overwhelmed by their caregiving responsibilities, and a few take advocacy beyond the personal level to the political.
