Early aggressive fluid resuscitation is necessary to treat intestinal mucosal hypoperfusion associated with shock states to avoid non-occlusive mesenteric ischemia.
Intestinal failure is defined as the reduction of functional gut mass below the minimal amount necessary for digestion and absorption adequate to satisfy the nutrient and fluid requirements for maintenance in adults or growth in children.
Stress-related mucosal disease (SRMD) is multiple superficial erosions occurring in the proximal stomach involving superficial capillaries secondary to mucosal hypoperfusion.
SRMD is commonly associated with UGI bleeding, but gastric perforations are rare.
Mechanical ventilation and coagulopathy are the two greatest risk factors for SRMD.
Acid suppression therapy should be instituted in all patients at risk for SRMD.
