Definition

The Craig Handicap Assessment and Reporting Technique Short Form (CHART-SF) is a 19-item measure of handicap or level of societal participation. Released in 1998, the CHART-SF is the short version of the 32-item CHART instrument designed to provide a simple, objective measure of the degree to which impairments and disabilities result in handicaps (societal participation limitations) for adolescents and adults (15 years and older) in the years after initial rehabilitation. Like its precursor, the CHART-SF includes sixsubscales (physical independence, cognitive independence, mobility, occupation, social integration, and economic independence), which closely reflect the disablement model developed by the World Health Organization, published in 1980 and revised in 2001.
