Zusammenfassung

Die Mortalität von kleinen Frühgeborenen konnte in den vergangenen Jahrzehnten immer weiter gesenkt werden. Allerdings werden zunehmend auch langfristige Folgen der Frühgeburtlichkeit evident. Die im Rahmen der sog. Barker-Hypothese postulierten langfristigen Auswirkungen widriger perinataler Umwelteinflüsse konnten mittlerweile durch zahlreiche Studien belegt werden. Aber auch unmittelbar in der Neonatalperiode sind Kenntnisse von Physiologie und Pathophysiologie der endokrinen Regulation wichtig, um Störungen der Funktion endokriner Organe beim Früh- und Reifgeborenen zu erkennen und diese adäquat zu therapieren. Das vorliegende Kapitel gibt einen Überblick über verschiedene Aspekte endokriner Funktionen und deren Störung in dieser besonderen Lebensphase, mit einem Fokus auf Wachstum, Glukose-Homöostase, Schilddrüsen- und Nebennierenfunktion sowie Kalziumstoffwechsel des Früh- und Neugeborenen. Dabei ist unser derzeitiges Verständnis der perinatalen Endokrinologie bei Weitem noch nicht umfassend oder abgeschlossen.
