FormalPara Synthese – Verteilung – Abbau – EliminationNach oraler Zufuhr beträgt die Bioverfügbarkeit wegen First-Pass-Effekt lediglich 50 %. Pethidin und sein hepatischer Metabolit Norpethidin werden hydrolysiert. Im Urin finden sich neben wenig Pethidin vorwiegend die Abbauprodukte. Norpethidin ist pharmakologisch aktiv.

FormalPara Funktion – PathophysiologieBei akuter Intoxikation finden sich Atemdepression, Koma, Bradykardie und Miosis.

FormalPara InterpretationTherapeutischer Bereich (S, P): 0,1–0,8 mg/L; toxisch: 1–2 mg/L; komatös/letal: >2 mg/L. Obwohl weltweit ein häufig angewendetes, starkes Analgetikum ist Pethidin nicht Bestandteil eines allgemeinen Drogenscreenings, sondern lediglich über Zusatzanalysen zu erfassen.
