Chronischer Schmerz bei Kindern und Jugendlichen

Zusammenfassung

Chronische Schmerzen im Kindes- und Jugendalter sind häufig. Für Deutschland geht man davon aus, dass etwa 5 % der Kinder und Jugendlichen mit chronischen Schmerzen stark in ihrer Funktionsfähigkeit eingeschränkt sind und die Kriterien einer Schmerzstörung erfüllen. Nach einem einführenden Fallbeispiel werden in dem Kapitel die diagnostischen Kriterien chronischer Schmerzstörungen, wie sie für das Kindes- und Jugendalter angewandt werden, vorgestellt. Dabei wird auf das ICD-10 sowie die aus dem DSM-5 resultierenden Veränderungen eingegangen. Im Abschnitt zur Demografie/Epidemiologie werden Häufigkeiten der chronischen Schmerzstörung berichtet. Im Abschnitt zur Ätiologie wird das bio-psycho-soziale Modell zur Entstehung und Aufrechterhaltung chronischer Schmerzen bei Kindern und Jugendlichen vorgestellt. Dabei wird insbesondere auf die Beeinflussbarkeit der Schmerzwahrnehmung durch psychische und soziale Faktoren und auf durch den Schmerz verursachte psychologische Veränderungen eingegangen. Für die Diagnostik werden verschiedene, im deutschsprachigen Raum häufig zur Anwendung kommende, Instrumente mit dem jeweiligen Anwendungsaltersbereich vorgestellt. Abschließend werden Behandlungsmethoden dargestellt, wobei ein Schwerpunkt auf psychologischen Interventionen sowie dem Einbezug der Eltern in die Behandlung liegt.
