Zusammenfassung
Das für das Verständnis der Pathogenese degenerativer Hirnerkrankungen so bedeutsame Prinzip der selektiven neuronalen Vulnerabilität wird dem Neurologen wohl bei kaum einer Erkrankungsgruppe so deutlich vor Augen geführt wie bei den Motoneuronerkrankungen. Basierend auf diesem topischen Prinzip lassen sich die Erkrankungen in den Typ der spastischen Spinalparalyse (Läsion der Betz-Zelle und der Pyramidenbahn), der spinalen Muskelatrophien (Läsion der motorischen Vorderhornzellen) und einer Kombination beider Läsionstypen – der amyotrophen Lateralsklerose (ALS) – einteilen. Die Ursachen der Erkrankungen der Motoneurone sind weitgehend unbekannt, und das Verständnis ihrer genetischen Grundlagen, der Pathogenese und die Entwicklung von Behandlungsansätzen stehen am Anfang. Dabei wird zunehmend klar, dass die mit einem monogenetischen Erbgang assoziierten Varianten zwar einen wichtigen Modellcharakter haben, sie aber allein nicht wegweisend für die so dringend benötigten therapeutischen Ansätze sind. Es wird heute häufig vergessen, dass auch die entzündlich und toxisch bedingten Erkrankungen der gleichen Systeme aufgrund ihres Modellcharakters ebenfalls einen Zugang zur Ätiopathogenese des charakteristischen Musters selektiver Vulnerabilität bieten.
