Zusammenfassung
Bariatrische Chirurgie umfasst verschiedene Operationsverfahren, welche das Köpergewicht von adipösen Patienten reduzieren. Wesentliche Wirkmechanismen sind Restriktion durch ein funktionelle Magenverkleinerung sowie Malabsorption. Hinzukommen kommen ausgeprägte neuroendokrine Effekte, welche zu einer Verminderung des Appetits und einer günstigen Beeinflussung des Energie- und Glucosestoffwechsels führen. Die sog. metabolische Chirurgie adressiert dabei vor allem den Typ-2-Diabetes mellitus. Als Indikation zur Durchführung der Operationen wird ein BMI von >35 kg/m2 angesehen, nachdem vorausgehende Gewichtsreduktionsversuche unzureichend erfolgreich waren. Zudem ist eine ausreichende Motivation des Patienten Voraussetzung, da postoperativ eine dauerhafte Anpassung des Essverhaltens und gezielte Einnahme von Mikronährstoffsupplementen notwendig ist. Alle operierte Patienten sollten in strukturierte Nachsorgeprogramme zur Verhinderung von Komplikationen eingeschossen werden. Trotz möglicher Komplikation ist heute erwiesen, dass die Operationen Morbidität und Mortalität von adipösen Patienten erheblich senken und die Lebensqualität steigern.
