Herzrhythmusstörungen bei Kindern und Jugendlichen

Zusammenfassung

Unter bradykarden Herzrhythmusstörungen versteht man eine temporär oder permanent abnorm verminderte Herzfrequenz unterhalb der Altersnorm. Bei symptomatischen Patienten ist die definitive Therapie die Herzschrittmacherimplantation. Tachykarde Herzrhythmusstörungen sind durch eine Erhöhung der Vorhof- und/oder Kammerfrequenz gekennzeichnet. Akute Episoden einer supraventrikulären Tachykardie können durch Vagus-Manöver, Antiarrhythmika sowie durch externe Kardioversion behandelt werden. Bei Kindern und Jugendlichen mit symptomatischen supraventrikulären Tachykardien wird die Katheterablation des anatomischen Substrats empfohlen. Akute ventrikuläre Tachykardien sollten primär durch externe Kardioversion terminiert werden. Zur Prävention des Wiederauftretens ventrikulärer Tachykardie kommen Antiarrhythmika, die Hochfrequenzstromablation des anatomischen Substrats der Tachyarrhythmie, chirurgische Eingriffe sowie die Implantation eines internen Kardioverter-Defibrillators zum Einsatz.
