How To Combine Adjuvant Chemotherapy and Radiation Therapy

Abstract

The use of breast-conserving treatment (combining breast-conserving surgery and irradiation, RT) had been well established based on the results of clinical trials demonstrating equivalent survival for patients with early-stage breast cancer treated by this approach or by mastectomy [1–5]. Breast- conserving treatment results in important gains in body image and sexuality for many patients and is now widely used in both the United States and Europe. Over a similar time frame, clinical trials have also demonstrated the effectiveness of adjuvant systemic therapies in prolonging survival [6] (reprinted in part in this volume). Therefore clinicians are now commonly faced with the necessity of combining RT and systemic therapy in patients following conservative surgery. In this chapter we review the available information regarding the integration of RT and chemotherapy (CT) following conservative surgery. We do not address issues related to the combination of RT and tamoxifen or the use of initial CT (discussed elsewhere in this volume).
