Zusammenfassung

Enzephalitiden sind eine Gruppe neurologischer Erkrankungen, die sich akut bis subakut durch eine Entzündung des Gehirns entwickeln. Bis vor einigen Jahren wurden die meisten dieser Erkrankungen als erregerbedingt angesehen, obwohl häufig kein Erreger nachweisbar war. Durch die Entdeckung verschiedener Autoantikörper gegen neuronale Rezeptoren, synaptische Proteine oder Ionenkanäle konnte in den letzten 10–15 Jahren eine Gruppe autoimmuner und weitgehend antikörpervermittelter Enzephalitiden identifiziert werden. Während Enzephalitiden mit typischen antineuronalen Antikörpern gegen intrazelluläre onkoneuronale Antigene, wie Anti-Hu oder Anti-Ma fast immer paraneoplastisch sind, liegt bei den hier beschriebenen Autoimmunenzephalitiden nur in einem Teil der Fälle ein Tumorleiden zugrunde. Diese autoimmunen Enzephalitiden werden in diesem Kapitel näher dargestellt.
