Angioplastie der infrarenalen Aorta abdominalis

Summary

We report the successful angioplasty of a significant atherosclerotic stenosis of the infrarenal abdominal aorta in five patients. They all had typical disabling claudication and one had an embolic event. We used the kissing balloon-technique three times, and in two cases, Trefoil-catheters. In two patients concomitant obstructions of the iliac arteries were treated successfully by angioplasty techniques. In all patients angioplasty was successful, and stenosis and pressure gradient were almost completely abolished. From a clinical point of view, all patients had a good longterm result (mean 25 months); in one case angiography showed a re-stenosis. In addition, no serious complication occurred. Percutaneous transluminal aortic angioplasty has proved to be safe and efficacious, and is the treatment of first choice in patients with atherosclerotic infrarenal aortic stenosis.

Zusammenfassung

Es wird über die erfolgreiche Angioplastie von signifikanten arteriosklerotischen Stenosen der infrarenalen Bauchaorta bei 5 Patienten berichtet. In allen Fällen lag eine typische Claudicatio intermittens vor; einmal hatte eine periphere Embolie auf die Erkrankung aufmerksam gemacht. Dreimal kamen die Doppelballon-Technik, zweimal Ballons vom Typ des Trefoil-Katheters zum Einsatz. In 2 Fällen wurden gleichzeitig Obstruktionen der Beckenstrombahn mit angioplastischen Verfahren behandelt. In allen Fällen war die Angioplastie erfolgreich, die Stenose und der Druckgradient wurden weitgehend beseitigt. Klinisch fand sich bei allen Patienten auch ein gutes Langzeit-Ergebnis (im Mittel 25 Monate), einmal war angiographisch eine Restenose anzunehmen. Ernsthafte Komplikationen wurden nicht beobachtet, so daß die Angioplastie bei der Stenose der infrarenalen Aorta als Therapie der ersten Wahl anzusehen ist.
