Summary

Only half of the operations performed for cancer have a curative conclusion. Patients with incurable diseases need palliative surgery with minimal intervention. Therefore, the use of minimal invasive surgical techniques should be discussed. In gastrointestinal neoplasms, especially of the stomach and colon, major palliative resections are often more successful in regard of postoperative quality of life. The surgical technique is only a part, but an important one, in palliative care. The most important aspect of palliative care is the improvement of quality of life. Highest priority is directed towards pain control. This goal can be achieved with individual decisions based on close communication with patient, relatives and specialists.

Zusammenfassung

Nur etwa die Hälfte aller wegen bösartiger Tumoren durchgeführten Operationen können mit kurativem Ziel beendet werden. Gerade bei unheilbar Krebskranken, bei denen der Eingriff mit palliativer Zielsetzung so klein wie möglich gehalten werden soll, ist der Einsatz der minimal-invasiven Chirurgie zu diskutieren. Bei Tumoren des Gastrointestinaltraktes insbesondere am Magen und im Bereich des Kolons sind jedoch auch palliative große Resektionen oft erfolgreicher hinsichtlich der postoperativen Lebensqualität. Die Operationstechnik ist ein wichtiger Teilbereich der Palliativmedizin. Wichtigstes Ziel ist für die letzten Lebensmonate eine Verbesserung der Lebensqualität. Höchste Priorität hat die Schmerzminderung. Dieses Ziel ist nur in enger Kommunikation und individueller Abstimmung mit Patient, Angehörigen und Fachkollegen möglich.
