Vascular Compliance in Heart Failure: A Contributor to Impedance, and the Response to Vasodilator Drugs

Abstract

The load placed on the failing left ventricle is an important determinant of its performance and thus a critical factor in the symptoms and progression of left ventricular failure [1]. The load imposed by the vasculature has traditionally been characterized by the calculated systemic vascular resistance, which merely represents the ratio of mean arterial pressure to flow. In a pulsatile system, however, the mean pressure and flow provide little insight into the dynamic nature of the left ventricle and the response of the vasculature to pulsatile pressure. To understand the total impedance to flow imposed by the vasculature, therefore, knowledge of vascular resistance, which is confined largely to the arterioles, must be supplemented by knowledge of the arterial compliance, which exists at all levels of the arterial bed [2].
