Abstract
This chapter focuses on intimate partner violence (IPV) as addressed by the healthcare system. IPV is associated with numerous physical and mental health outcomes that require survivors and perpetrators to seek medical attention more frequently than others. As such, healthcare systems, such as primary care, emergency departments, and other medical subspecialties, are often the first point of contact for IPV survivors. Further, the reduced sense of stigma and increased privacy afforded by health settings uniquely positions the healthcare system as a safe space for IPV inquiry, detection, and early prevention and intervention efforts. Yet, both lifetime and current experiences of IPV victimization are often underreported by survivors and overlooked by providers in healthcare settings. While many have called for better training of individual healthcare providers, more recently, healthcare systems have been tasked with systematically recognizing and responding to IPV in a coordinated fashion. Consequently, this chapter will review: (i) primary and second-hand exposure to IPV and its consequential effects on health, (ii) healthcare utilization and economic impacts associated with IPV, (iii) best practice recommendations for screening for IPV as well as for clinical responses following disclosure within healthcare settings, (iv) barriers to implementing screening in the current healthcare system, (v) evidence related to patients’ responses to screening, (vi) five proposed models of system level change and (vii) current efforts to move beyond screening toward universal prevention and intervention practices. Finally, this chapter will describe current models and proposed pathways to IPV prevention and intervention via coordinated, evidence-based, involvement throughout the healthcare system.
