Alternative Treatments for Local Anesthetic Systemic Toxicity: Therapeutic Hypothermia After Bupivacaine-Induced Cardiac Arrest

Abstract

Ultrasound-guided peripheral nerve blocks have revolutionized regional anesthesia. Considered safe, regional anesthetic procedures still can be complicated after accidental intravascular injection of local anesthetic. The devastating effects have been described as local anesthetic systemic toxicity (LAST). The preferred antidote is intravascular lipid emulsion. Therapeutic hypothermia which protects neurological and cardiac function also may offer benefit in LAST, especially in situation where the lipid emulsion is not readily available such as in remote locations and frontline combat hospitals.
