Abstract

Esophagogastroduodenoscopy (EGD) is the diagnostic imaging procedure of choice in initial step of diagnosis of gastric carcinoma. EGD is a highly sensitive and specific diagnostic test, especially if combined with endoscopic macrobiopsies and histological examination of the tissue. Today technological advances permit a better diagnosis of tumors, thanks to the use of different technologies in combination with endoscopy. Some examples of new technologies are chromoendoscopy and magnification endoscopy that are useful to improve visualization of mucosal details, narrowband imaging that evaluates the capillary pattern correlated with the degree of parietal infiltration of the neoplasm, and last but not least confocal laser endomicroscopy that permits in vivo analysis of tissue microarchitecture, with cellular resolution, thus allowing a precise identification of areas to be biopsied and an evaluation of neoangiogenesis. The endoscopic diagnosis of early gastric cancer (EGC) requires endoscopic techniques that follow quality standards and thorough knowledge. Moreover, endoscopic ultrasound (EUS) allows more accurate locoregional staging of early or locally advanced gastric cancer.
