Radioguided Parathyroidectomy

Abstract

Radioguidance for parathyroidectomy has broad applications and is often underutilized in parathyroid surgery. By using a low dose of technetium 99m sestamibi, injected the day of surgery, radioactivity can be detected within the parathyroid tissue at time of surgery. This functions to help localize glands during surgery, confirm that excised specimens are parathyroid tissue, and give an assessment of gland metabolic activity. It should be considered one of a parathyroid surgeon’s tools, along with intraoperative parathyroid hormone monitoring and preoperative imaging. We find that radioguidance is helpful in all parathyroid cases, from minimally invasive to reoperations to bilateral neck explorations.
