Abstract

‘The last one hundred years have seen a greater improvement in health than at any time in the last three millennia’ (WHO 2000: 4). The evidence shows that not all countries or individuals have benefited equally, the poorest 20 per cent of people in the world are approximately ten times more likely to die before the age of 14 than the richest 20 per cent, whilst more than 90 per cent of maternal and child deaths occur in developing countries (DFID 2000a: 8, 2000b).
